Sorry guys for this, but I tried to use common sense to find the location of San Juan. Please don't ban me or anything. Just wait.
San Juan is to the west of Palmont City, why:
1- San Juan was supposed to be part of MC and MC is to the northwest of Palmont.
2- It wouldn't make sense to me if SJ was in MC and Lookout Point was beside MC. So, it doesn't make sense to me for the player to go all around Palmont with Nikki to SJ if it's to the east of Silverton.
3- If you notice that after the tutorial in the beginning, the player heads east with Neville. Why do I say this? Because if you look in the background behind the player, you can see city rays, which MW says its Silverton. Plus, the player didn't turn around, he just continued east.
So, I think that SJ is to the Northwest of Palmont, behind the mountain beside MC.
